Sport physicals at St Claire Family Med, across from High School on August 20th starting at 4:00 pm. Parent needs to call 738 5155 for an apt. Parent or legal guardian must accompany student under 18 years of age.
History Part of Physical form needs to be filled out before apt time. Forms available in School offices or on the Elliott County homepage under menu> documents>Health>Athletic Physical Form History/Exam.
Coaches will need consent pages also.
Sport physicals are due yearly.
